What does GUIDE provide?
Through GUIDE, eligible families can access:
A dedicated care navigator who helps coordinate care and connect you with resources
24/7 support line for caregiving questions and urgent concerns
Caregiver training and education specific to dementia care
Respite services so primary caregivers can take a break
Who is eligible?
GUIDE is designed for people who:
Are enrolled in Original Medicare (not Medicare Advantage)
Have a dementia diagnosis
Live in the community (not in a nursing home)
What does it cost?
There is no out-of-pocket cost for families. GUIDE is funded by Medicare.
