The GUIDE Program offers support designed to ease the burden of dementia caregiving and improve quality of life for families and their aging loved ones. Here's what it includes.
Care navigation
A dedicated tendercare Guide, who serves as your GUIDE care navigator. They coordinate between doctors, caregivers, and support services so everyone stays on the same page, and they're your point of contact throughout.
24/7 support access
An around-the-clock helpline to reach your care team whenever you need assistance.
Personalized care plan
A care plan developed and regularly updated around your loved one's individual needs.
Assessment
An evaluation of clinical, behavioral, and psychosocial needs.
Respite care
Up to $2,625 per benefit year, used for in-home care or facility-based services.
The benefit year resets July 1, and unused funding doesn't carry over. See Respite care in the GUIDE Program.
Caregiver education
On-demand training on dementia care, medication management, fall prevention, and more.
Community resources
Access to local support services including transportation, meal delivery, and financial aid.
Ongoing monitoring
Regular check-ins and proactive adjustments to the care plan as your loved one's needs change.
